Hamilton Centre - v1.5

The Hamilton Centre Clinical Network is a collaborative initiative where providers work together to improve access to integrated mental health and addiction treatment. 

The network is formed by some of the most respected public healthcare organisations in Victoria - St. Vincent's Hospital Melbourne, Eastern Health, Western Health, Austin Health, and Goulburn Valley Health. Its primary objective is to improve outcomes for individuals with co-occurring conditions by supporting service providers in delivering integrated care.

Consultation services are provided through both in-person and telehealth appointments, accessible across metropolitan, rural, and regional areas of Victoria.

The Centre's Service Navigation Team supports and mentors clinicians in integrated care delivery, with service navigation and linkage, information provision, and requests for education or training throughout the state.
